Total Shoulder Replacement: Alleviating Pain and Restoring Shoulder Function

Understanding the Shoulder Joint

The shoulder is a complex “ball and socket” joint comprised of two main parts: the humeral head (the ball) and the glenoid portion of the scapula (the socket). Generally, a thick cartilage cap surrounds the bones on either side of the joint, providing smooth and pain-free movement. However, over time, this cartilage can wear down, leading to various stages of arthritis. When the joint is fully worn down, the cartilage cap disappears, resulting in “bone-on-bone” arthritis as seen on X-rays.

When to Consider Total Shoulder Replacement

When conservative measures no longer control pain or improve function, a total shoulder replacement (also known as total shoulder arthroplasty) may be recommended. This surgical procedure aims to alleviate pain and restore function by replacing the damaged joint components with artificial implants.

What is a Total Shoulder Replacement?

Total shoulder replacement involves removing the arthritic parts of the shoulder joint and replacing them with size-specific implants. Here’s an in-depth look at the procedure:

  • Incision and Access: The surgery begins with an incision made on the front of the shoulder. The subscapularis muscle is reflected, allowing access to the shoulder joint.
  • Removal of Arthritic Components: The arthritic humeral head is removed and the damaged glenoid socket is reshaped.
  • Implant Placement: The humeral head is replaced with a combination of Cobalt-Chrome and Titanium implants. The glenoid side is reconstructed with a Vitamin-E infused polyethylene component.
  • Evaluation of Joint Stability and Motion: The stability and range of motion of the joint are evaluated to ensure proper function.
  • Subscapularis Repair: The subscapularis muscle is repaired, a crucial step for the success of the operation.

Post-Operative Recovery

Post-surgery, the shoulder is immobilized in a sling for six weeks. Recovery follows a phased approach:

  • Phase 1 (0-6 Weeks): Self-directed rehab begins while the shoulder remains in a sling.
  • Phase 2 (6 Weeks – 3 Months): The sling is removed, and focus shifts to regaining range of motion.
  • Phase 3 (3-6 Months): Strengthening of the shoulder begins.
  • Long-Term Recovery: Most recovery occurs within 3-6 months, with continued improvements expected up to one year following surgery.
